At Forma Chest, we are committed to your satisfaction and successful treatment journey. Please read this policy carefully before making a purchase.
30-Day Return Window
We accept returns within 30 days of the delivery date. To be eligible for a return, your item must be unused, in the same condition that you received it, and in its original packaging. Vacuum bell devices must show no signs of use for hygiene and safety reasons.
Non-Returnable Items
- Used vacuum bell devices (for hygiene and medical safety reasons)
- Replacement parts that have been opened
- Digital products or downloadable content
- Items purchased as part of a final sale
How to Initiate a Return
To start a return, please email us at support@formachest.com with your order number and reason for return. We will respond within 2 business days with a Return Merchandise Authorization (RMA) number and return instructions. Do not send items back without an RMA number — such shipments will be refused.
Refund Processing
Once your return is received and inspected, we will send you an email notifying you that we have received your item. If approved, your refund will be processed and a credit will automatically be applied to your original payment method within 5–10 business days. Shipping costs are non-refundable.
Exchanges
If you need to exchange an item due to a defect or damage upon arrival, please contact us at support@formachest.com within 7 days of delivery with photo evidence of the damage. We will cover the cost of the replacement and return shipping for defective items.
Late or Missing Refunds
If you have not received a refund within 10 business days of our approval notice, please first check your bank account, then contact your credit card company as it may take additional processing time. If you have done all of this and still have not received your refund, contact us at support@formachest.com.
